The National Instant Check System (NICS) is only as good as the states' contributions to the database.  Many states do not submit information to NICS or do so rarely, especially derogatory mental health adjudication records.  The National Shooting Sports Foundation (NSSF) has had a "Fix NICS" program where they are trying to get states to submit of their applicable records which seems to have finally taken off in the last few years.
